What information needs to go on an emergency card:
- Who is your emergency contact person? How do first responders reach that person? Are there other people to reach out to if your first emergency contact person does not respond to calls?
- What medications are you currently taking, if any? Some drugs interfere with others or cause adverse reactions when mixed with other medicines.
- Do you have any medical conditions first responders should know about when caring for you? For example, if you have diabetes or if you are allergic to certain medications, medical teams need to know.
- What is your blood type? The doctor’s office or the Red Cross can tell you if you have ever donated blood.
- What is your doctor’s name? What is your doctor’s contact information? Contacting your doctor to get specific information about your medical history can often be an essential step in caring for you.
- Do you have a religious preference or not? Mark this if you want caregivers to know.
- Do you want to donate your organs? Letting first responders know this can be important in helping you carry out your wishes to help others.
- What documents have you put in place? For example, do you have a Health Care Proxy, a Power of Attorney or a Living Will? Loved ones can use these documents to avoid going to court to try to make the decisions you might want to be made on your behalf.
- Who is your insurance provider and what is your member or policy number? Making sure your insurance provider knows that something has happened within a certain time period is often mandatory for health insurance to cover hospital stays or other medical intervention like ambulance services.
